For your visit, please bring any eyeglasses you currently wear with you, including computer, reading or prescription sunglasses.

Bring any eyeglasses you wear.

If we dilate your eyes, be aware that your vision may be blurred and light-sensitive for 2-4 hours afterwards.

If you have sunglasses, please bring them with you.

Please fill out the NEW PATIENT HEALTH HISTORY FORM and share with our office prior to your appointment. If that is not possible, please bring the completed form with you to your visit.

If you are interested in computer glasses, please fill out the COMPUTER QUESTIONNAIRE FORM and bring it with you to your visit.
